All new patients are asked to complete our patient information forms prior to the first in-office or Teledentistry appointment. Periodically, established patients will be asked to update health history, so that records remain current. For your convenience, all of our forms are available by clicking on the following links.
Fox Dentistry does not participate in dental insurance plans. If you do have dental insurance, we are happy to complete any paperwork to process your claim. You are responsible for the cost of your treatment, payable on the day of service. Payment plans are available in certain situations. Our office accepts cash, check or credit cards. We can also direct you to additional credit options if requested.
After you leave our office, you may have questions you forgot to ask during your appointment. Please visit our Frequently Asked Questions page for dental issues. If you are considering making Dr. Fox your dentist, we offer a short Quiz that will help you learn if Dr. Fox is a good match for your oral health needs. Let us know how we can assist with additional information about dental health or Fox Dentistry. We are here to serve you.
